Draft of Technology Committee survey to SPA chairs

This is the survey we would have placed online in MySFA.

We will only do ONE this year (professors). Next year we can survey Teacher Candidates and the following year, we can plan to survey our teachers in the field and once they land a job. We can talk about this later.  I thought it best to keep it simple but if you think SPA chairs can manage data collection for professor and TC, then let me know.
